ADMISSION REQUIREMENTS:

A. for Incoming Freshmen

  • Form 138 (High School Card)
  • Certificate of Good Moral Character
  • Birth Certificate (Photocopy with NSO Authentication)
  • College Entrance Test


B. For Transferees

  • Honorable Dismissal
  • Informative copy of Transcript of Records
  • Certificate of Good Moral Character
  • Birth Certificate (Photocopy with NSO Authentication)
  • College Entrance Test


Additional Requrements for enrolees in the following courses:

BSMT. BSMarE, and SRC

  • Ishihara test result from DCSP clinic
  • Hearing test result from DCSP Audiometric Lab.
  • Medical Certificate (any licensed physician)


BS Nursing

  • Hearing test result from DCSP Audiometric Lab.
  • Medical Certificate (any licensed physician)
